Warrants and Orders of Protection: How To Get a Warrant For a Worthless Check
At the District Attorney's Warrants Screening Office, located at the Metropolitan Justice Center Detention Facility

(2nd Avenue North between James Robertson Parkway and Gay
Street) between the hours of 8 a.m. and 11:30 p.m. Monday
through Friday, excluding holidays.

WORTHLESS CHECK
The following information must be provided before the Court can issue a warrant for a bad check violation:
- Name of victim (If recipient of check is a corporation, give corporate name; if recipient of check is a partnership, give names of partners and business name; if recipient is a sole proprietorship, give business name and name of owner)
- Name, address, and phone number of employee who accepted check
- If a photograph is available of the defendant
- If a handwriting analysis has been done, and if so, by whom, their address, and telephone number
- Date notice was sent to the defendant
- Certified receipt of Demand for Payment letter
- Name, address, social security number, and drivers license number of the defendant
- Description of the defendant
- What item(s) were obtained with the check
- Name, address, and telephone number of the person who approved the check (if different from the person who accepted or cashed check)
- Names, addresses, and telephone numbers of persons who can identify the defendant
- Any other pertinent information
Also bring with you a self-addressed, stamped envelope

Once the summons has been issued, DO NOT ACCEPT ANY CASH UNLESS
OR UNTIL THE SUMMONS HAS BEEN CLEARED
