Inspection

The Safety and Inspections Division is organized to include the following sections and units:

This is being provided as a reminder of what to do in the event of an Injury on Duty (IOD):

Civil Service rules require that the employee give notice to their supervisor immediately. The supervisor should review the circumstances surrounding the injury and be fully aware of how the IOD occurred. The supervisor makes the initial determination of an IOD and authorizes any emergency medical treatment. The supervisor will document this with the completion of Form 101. This form must be faxed to ASC within 24 hours of the supervisor's notice of the injury. The fax # 1-877-669-9140 is in the upper right hand corner of the form. There is also another alternate fax # 1-775-908-3724 in the upper right hand corner of the form. Since ASC handles the coordination of medical treatment and the payment of medical expenses this fax insures that our employees get the medical attention they need, and that if contacted by a medical provider ASC is able to immediately authorize treatment. The original of the 101 should be sent to the Safety Office immediately after it is faxed to ASC.

Supervisors should have Employer's Authorization for Care (Red Cards) to give to the treatment provider that authorizes them to provide emergency care, and makes certain that billing is sent to ASC. Red Cards can be obtained from MPD Supply.

The employee will also need a 201 IOD Report Form completed by the treating physician, and must be completed by the physician at any subsequent appointments. This provides the necessary information on the employee's injury and their work status. The 201 from can be obtained on the PDWEB under Forms/Safety/Metro Work Status.

If a prescription is needed in connection with the IOD a 202 Pharmacy Authorization Form must be completed and signed by the supervisor. This form will be provided to the pharmacy where the prescription if filled. The 202 form can be obtained on the PDWEB under Forms/Safety/RX Authorization.
